Tennessee World War I questionnaire. As noted, Hubert was sent this by the State of Tennessee and then returned it for their documentation project.

Three of these pages are in excellent shape and are very readable. The last - and perhaps most interesting page - is not. I have attempted to improve the contrast and visibility, but there must of have been damage to the original. I have also added text to the last page in which the battles in which Hubert fought are noted. If one is interested, an online search for the "Old Hickory" Division, Tennessee, WWI will generate a number of hits. Note the areas listed on page four in which Hubert served.
